MEET FORMS & SANCTION APPLICATION

Effective September 8, 2023 all applications for sanctioned or approved events in the Wisconsin Swimming LSC must be submitted electronically.  All applications must comply with USA Swimming and Wisconsin Swimming rules and policies as of the date of application. All submissions must include an application form and a proposed meet packet. Use the link below to submit your applications and packets.

MEET HOST FORMS and REPORTS

  • Coach Sign-In Sheet
  • Meet Financial Report - DUE to LSC within 7 days of meet end date
    • ​At the November 2023 LSC BOD meeting, the Board voted to approve the assessment of a $50 sanction/approval fee for all meets, regardless if individual event fees are charged or not.  The sanction/approval fee would be collectible starting January 1, 2024.
  • Meet Report Form - DUE to LSC within 7 days of meet end date
