MEET FORMS & SANCTION APPLICATION

Effective September 8, 2023 all applications for sanctioned or approved events in the Wisconsin Swimming LSC must be submitted electronically.  All applications must comply with USA Swimming and Wisconsin Swimming rules and policies as of the date of application. All submissions must include an application form and a proposed meet packet. Use the link below to submit your applications and packets.

MEET HOST FORMS and REPORTS

• Coach Sign-In Sheet - All USAS coach credentials should be verified at sign in
• Meet Financial Report - DUE to LSC within 7 days of meet end date
• Meet Report Form - DUE to LSC within 7 days of meet end date

MEET BIDS and ADDITIONAL MEET FORMS

• Meet Bid Form - Short Course Meet Bids are no longer being accepted.  Please contact meets@wisconsinswimming.org to add a late bid

• Championship Meet Bid Form - Bids for 2026 Championship Meets are no longer being accepted.

SWIMS TIMES REPORTING FORMS

• Request to manually load Observed Meet times into SWIMS - Form "C-2" - Use this form only if a swimmers times are NOT in SWIMS
